Clean URLs for custom query variables in Wordpress

Project Description:
I have URLs involving custom functionality in this form:http://mysite.com/{any path}/?myvar=foo
I need them to be accessible like this:
http://mysite.com/{any path}/my/foo
The solution needs to work for all paths (posts, taxonomies, archives, anything), for any permalink structure. It may involve .htaccess, but will also require Wordpress code.
It looked simple enough, but I am just getting 404 errors when I try, so I'm posting here.
