竞赛简介
Simple 6 page website that will save data each timer a screen is completed. Will collect information for providing an insurance quote. First screen will be collecting general info on the user. Second screen will be to collect more detailed info on the user (data carried over from first screen). Third screen will collect data on additional drivers (and can happen multiple times). Fourth screen collects data on primary vehicle. Fifth screen collects info on additional vehicles. Sixth screen is a simple thank you screen.
Message will be sent to a specified email account whenever a user either completes or abandons the app. I have purchased a vehicle database that lists year, make, model and trim to be used to populate drop downs on the vehicle screens.
The attached shows the content of each of the screens.
I have added a zip file with a logo for you as well.
Overall guidelines:
1. Site should be PHP code working with a My SQL database
2. All source code will be the property of the buyer and shall be provided by developer
3. Test site and credentials will be provided by seller on hosting platform
4. As the user presses any buttons on the following screens, data already collected is to be saved.
5. Year, Make, Model, Trim to be pulled from MySQL database provided
6. Additional fields to be captured and saved - IP address, Browser version
7. Site should be ready for Google stats.
8. Each page should have a link to terms and conditions page
9. Each page should have link to Privacy Policy page
10. Terms and Conditions, Privacy Policy and Phone disclaimer should all be pulled from MySQL database fields
