Contact Form - JQuery & Ajaxified

I am using a simple coming soon template I purchased (here's a link to the demo: [login to view URL] ). The template has an ajaxified JQuery form which allows someone to submit their email address and the form will post to either CSV or email. You can see that the form has a JQuery effect which allows the form field to fade out on submit and replaces the area with a content Div that says something like "Thanks for submitting your info". Here's what I would like to do: Edit this form so that there is a Textarea comment field. On submit it would use the same Jquery effect to replace the comment field with the original email field. On submit of the email field it would be replaced by the content Div. So, basically I just want to add a comment text area with submit before the email field shows. The information can submit separately (twice) to either csv or by email. I don't care, the comment and the email address do not need to submit together. I've tried to hack this form and can't make it do what I need. Hope I've described well enough what I want to do.
