Build a Website and iPhone Application

I need to build a custom website to collect data.
This will be a short questionnaire that will be dynamically driven from the database.
This should be built in ASP.NET(C#) and SQL Server.
Along with this I need an IOS app to run the same questionnaire on an iPad that has no internet connection, retrieving the questionnaire when connected and storing the answers locally until connected then uploading them to the server.
There will need to be some nice transitions between pages/questions done with CSS/jQuery.
Additional Info will be provided to select bidders for more accurate bids.
We have a pretty short time frame to get this implemented so I need someone that can really hit the ground running and get things done.
Must speak English.
