Data Entry - Conversion of Formulas from PDF to HTML

For this project I'm trying to convert a list of ~500 formulas from a pdf file:

Into a html format as per:
[login to view URL]
For e.g. on page 3 of the PDF, the first formula is:
"The Future Value of a Single Cash Flow
FVN = PV (1+ r)N"
That would need to get converted into:
The Future Value of a Single Cash Flow: $\text"FVN" = \text"PV"(1 + r)^N$
The purpose is to create an online database of formulas which can be easily viewed and rearranged by users.
There are different ways of approaching this issue and I'm looking for someone who understands what I am trying to accomplish. Attention to detail is key, please contact me for further details.
