customer information application

I will need a desktop application created to handle the book keeping of a clinic. This will be used as an interactive calender for 2 seperate employees (with individual logiv) where both can make bookings for appointments and have them entered in their individual diary. it will also need to remind each customer on the morning of the appointment via emal as well as the employee 30 mins before the appointment on the application via a notification. I will need to contain a chart on each patient which will automatically be displayed before the appointment. this chart will contain a contact details information section, as well as diagnosis page, a treatment page and a reciepts page of past appoinments, All of this information will need to be stored in a database where each client (upto 20) will have its own file. please see proposed screenshots
