Design Tri-Fold Brochure

I need a trifold brochure designed to fold and print correctly. Custom icons and images also need to be created. I will need the Illustrator or Indesign files once completed and a PDF version of the file as well.
I have uploaded and example brochure that I would like to copy. I have also uploaded a mockup layout for my flyer. I would like to replace the solid colour of the example brochure to have outline colour as shown in the mockup. I will also provide in a word document the appropriate text to go in each section.
I need new icons created on the front page that better represent what is currently there.
