Design a website for conducting Lighting surveys & develop a database

I am looking for a website for making lighting surveys online using iphones, ipads, android, tablets, laptops etc. if 3 g/wifi is available the survey will be uploaded as the survey is completed, and saved at the end of the process, or if no 3g is available, when the devices comes into a wifi area, it will automatically upload the surveys to a central db and application so that we can make changes to the survey back in the office. I have attached a pdf that explains what I am trying to achieve, DB will contain, all information, we will need to be able to access the db using a web browser, the db can be hosted in our office or online server. website must look professional, designed and finished properly, We must have a system where we can update, add and amend all lighting and all parts of the db and to offer an update. Please review the attached pdf and provide an accurate bid and an maximum development schedule, we will not allow the project to over run. Milestones can be created but will only be release when we have the website uploaded to our server, not on our server working then no milestone will be released . all the reports that are in the pdf must be created, reports are exported as PDF and Excel docs that can be printed or emailed.
