Design an Enrollment / Application System UI

We are looking for a UI designer to develop a web user interface for an insurance enrollment system we are building. The UI design should have a clean, modern look that would work with a number of different brands. It should be designed in a way where major elements (colors, logos, etc) can be swapped out from one product to another using the system. We will also want a design that degrades well for mobile browsers (tablet and phone). We have resources that can take a design target and do the actual coding, but if you would like to include the cost of also doing this in your bid, feel free and do so - just let us know that is the case so we can use that in considering. I have attached a wireframe mockup with an example page for the enrollment system. The attached wireframe is just an example layout - and we are open to a different layout using the same elements if you have a better idea. The selected designer will need to design a 10-15 pages based on the final design (all with similar elements), as well as example layout variants for tablet and phone, and some example variants of selected pages showing swapped color and logo design elements. Expected deliverables would be layered PSD files. If also doing the coding, html/css/js as well.
