Guys and girls I have a comprehensive sign-up form that I would like built for me. It would be great if this form can write straight to a database for anyone that fills it out and sends it via email to an address I will add to the form. The database part is not highly compulsory but it would make life a little easier if the function could be built in. I will also need some sort of a reminder to be sent (possibly in e-mail form) when the entry reaches 11 months of existing on the database. The reminder can be sent using a flag. Eg. If someone signs up, that confirmation may be used as the flag to send an email reminder to me in 11months time. In Summary: *Comprehensive HTML form to be created (Fields or HTML markup will be provided) *PHP/Form to write info to a database (Indicate if this is included in your bid) *Email reminder to be sent to admin 11 months after user has signed up with a reminder for renewal *Email to be sent to admin as soon as the form is filled out and sent *Email to the customer confirming that we are busy with their application *Some sort of security measure added to the form to prevent SPAM *Any suggestions from you to make this process better I look forward to your bids. Hello, I can make a form with custom fields as your require. I can make a database for you and a backend system where you can check all the new entries in the forms and also support it by email notification system on both side. I would be using NodeJS server and MongoDB for that (and we can use Heroku free plan/hobby plan as required) I can even make a system on the backend, from where you can send a mail directly and can place checkmarks on the mails you had replied or so (but that would need extra time, hence extra work).
