Build a form and report builder app.

Part of a learning and development team in a support centre. Currently, instructors fill out a 'report' on learners when they complete a program. The report consists of 5 questions. Instructors manually fill out the report, and email them to the learners' lead. Instead, I want Instructors to fill out a form (scale of 1-4) where they quickly choose a score for each of the 5 questions, and have the option to write more below. Importantly, I want them to be able to fill out a field with an email address for the learners' lead. When they submit, the recipient (lead) should receive a nice report with the data, not the raw form. Looking for the following: - need a form built to ask questions on a 1-4 scale, with room for short answer for each question (this part is easy - could build in JotForm or Google Forms but...) - need a report that will be delivered to the recipients of our choice, which will change from report to report; need this report to be customized w/ our graphics & logos Use case: - instructor on my team will open the form and populate it with scores & short answer information - instructor will fill out a field with the recipient address and submit - instructor and recipient will receive final form and can save (to PDF) if necessary - instructor should also receive raw spreadsheet data of scores.
