Create simple app for android.

All of our new customers need to fill out a document and sign. We want to create an app for use on android-tablets for filling out this, instead of paper. ## Deliverables When app is started: choose between English and Norwegian flag. **Screen#1:** First screen they fill out personal info (6-8 fields). Button "continue" to next step **Screen#2:** a "yes" or "no" question. When choosing no: "continue" button is shown. When choosing yes: 23 lines with a check-field where they can check. Continue button is shown the whole time. **Screen#3:** a "yes" or "no" question. When choosing no: "continue" button is shown. When choosing yes: 7 lines with a check-field where they can check. Continue button is shown the whole time. **Screen#4:** a "yes" or "no" question. When choosing no: "continue" button is shown. When choosing yes: 4 lines with a check-field where they can check. Continue button is shown the whole time. **Screen#5:** 10 lines with a check-field where they can check. Continue button is shown the whole time. **Screen#6:** 4 questions, each with a text-field where customer can write. **Screen#7:** a "yes" or "no" question. When choosing no: "continue" button is shown. When choosing yes, text-field appears with text above "Please specify". **Screen#8:** 1. question with 3 answer possibilities (check fields). 2. Text-box with a text-line above. 3. Continue-button. **Screen#9:** Text that says "thank you. Please give the tablet back to reception". "OK"-button or something, that makes it come to the first screen again. All this must be saved in 1 A4-page PDF-file, so we can print or save it on computer/server. Name of PDF should be the name filled out in screen 1. Want input on a good solution: Not sure how it can be done easy to print/save the PDF on computer-network, as we don't want all this personal info saved on the tablet itself, in case maybe someone steals it or something like that. All text will be given in Norwegian (language has some special characters that is often used, and needs to work in the app) and English. Also needs to work on android-phones.
