A bespoke iOS application with approx 4 data capture screens (~20 fields), and a checklist screen (many checklists exist with a standard response (e.g. yes, no or Agree, Do Not Agree) type per checklist). Two-way sync of data from an existing web application. App to collect data to a single web server.

1 Statement of Requirements
1.1 General Requirements:
• The solution will need to allow data capture and review from the following mobile devices:
o iPhone,
o iPad,
• Capture of data “offline” (no cellular signal or Wifi connection) and then submit when “connected” (track this process via status)
• Capture directly when “connected”
• Store and recall previously submitted items (non-editable)
• Registration process as multiple customers will use the application and these need to seperated out.
1.2 Submission Stage
• Submission includes the capture of the following information:
o Event type (list) (unique per customer - will be 'pushed' to a table)
o Event Sub-Type (list) (unique per customer - will be 'pushed' to a table)
o Restricted (checkbox)
o Date of Event (date)
o Time of event (time)
o Date Reported (date)
o Time Reported (time)
o Work Area (text)
o Country (list) (unique per customer - will be 'pushed' to a table)
o Responsible Organisation (list) (unique per customer - will be 'pushed' to a table)
o Description of Event (text)
o Describe the event in sufficient detail (long text)
o Immediate actions taken (text)
o Actual Consequence (list) (unique per customer - will be 'pushed' to a table)
o Potential Consequence (list) (unique per customer - will be 'pushed' to a table)
o Potential Likelihood (list) (unique per customer - will be 'pushed' to a table)
Notes:
In Scope:
• Capture of GPS coordinate > Selection from a map.
• Capture of photographs from device. Capture from the camera or from the stored camera roll.
