apple mail applescript add-on OS X 10.10

need a preferably applescript apple mail add-on (osx 10.10) reachable from mail menu or with other easy access from mail app, with the following function:

-textbox for base email folder path, default value stored

-textbox to enter a variable

-ok button

-creates a (smtp) mailbox under base email folder path/ 'variable'

-sets up a rule to auto move email subject containing the 'variable'

code needs to be transparent and reviewable
