Create a PDF fillable form from scanned images

I have two scanned forms that need to be redesigned in Indesign or in PDF. The finished product would be exactly like the original scanned forms but in PDF fillable format. Since the original document attached were very thin, you can see some text bleeding from behind. This is the only reason I want to have it redesigned so please ensure that redesign looks like the original to the best Project Completion The project needs to be completed with in two days. (By end of Tuesday Canada Eastern Time) . Project Cost: This is easy for someone with right tools, there are total 4 pages. My budget is $65. however I will pay upto $100.00 for an excellent job. Project Payment Condition: I will pay you :) when the job is completed and within time and the bonus if you exceed my expectations and or the project fits the criteria. I am not very picky but the finished product must look like the ones i have attached. Thank you
