Cloud based Medical Patient Management system (EMR/EHR) - repost

I'm looking for a full site developed in PHP, & mysql, The site should be exactly as [login to view URL] please log in there and review each and all of their features. Since the project will consist in developing a site with all of their features and adding some more features that I will provide in the PMB process. Is vital that you understand all of their features. Modules should include: Patient records & history. Doctors appointments scheduling Front desk management E-mail confirmations and scheduling reminders to patient as well as medication dose reminders via e-mail or SMS In home appointment scheduling for patients Patients messages to Doctor/ Front desk assistant. Patient input from home of data such as Blood pressure, Glucose levels, weight, height.... uploaded to patient record data. Ability to print Prescriptions, request of Letters to doctor/ Immunization records request/ automated email reminders of upcoming appointments, Birthday letters/ anniversary reminders/ scheduled reminders/ For doctor/ Physician admin page: Doctors would be able to accept new patients only if patients Identity its confirm via 3 ways: Third party website/ SMS confirmation code/ Email ( will provide more details in PM) Patient dashboard of Key metrics on a single page ( will provide more details in PM) Reports of Visitors per day/week/month Report of Income per day/week/month Reports on Average wait time per patient Reports on Employee attendance list ( All employers need to log in to register their attendance and also their absence) Reports on Pending appointments per day/week/month Site will be developed in English but once its ready, I will provide translation terms via .INI file. Pharmacies/ Laboratory admin page: Both pharmacies and Laboratory would be able to "bid" or send "quotations" to patients based on Doctor prescriptions/ test required via Patient could receive "quotations" from Pharmacies/Laboratories based on "geo-location" ( pharmacies near Doctor practice/ Near patient Home/ between Doctor practice and patient home. Bidding process between pharmacies/ laboratories to "attract" patients based on price/quality/delivery of supplier. Entrainment Message board development: On screen message board for practice office in which patients can see Waiting time/ News/ weather/ Videos/ local interest news/ directory. two types of boards will be required: a) For Practice Waiting room b) For Hospital lobby ( I will provide details in PM) Hospital Nurse station: Messaging station vital signs recording ( updating medical charts of patient) Emergency contacting of doctor via SMS "out of range" warning system to Doctor messaging board. Super Admin Module: ( for me) I will provide more details in PM. I expect from this development a current technology features such as "facebook connect" Scope: Some of these features will be available to physician free, but some other features will be require a paid subscription via paypal subscription process.
