Convert PDF Form to a Web Form

This project requires the coder to convert the attached PDF form and create digital versions by creating ASP.NET web pages that will capture all the required data and produce exact duplicates of the hard copies with the data filled out on the form. Below are the specific requirements: 1) This project must be done in Visual Studio 2008 with Service Pack 1 and support the .NET Framework 3.5 runtime. 2) Exact duplicates of each page of the form should be drawn in Active Reports and each screen should collect the required data, create a pdf version of the form with data, allow the user to save a PDF version of the completed form, and allow the user to email the form to the recipients listed in the [login to view URL] file. 3) The aspx forms must capture data in the most intuitive and easy way for the end user. This means that sections of the aspx form that require repeated data must be captured using some type of repeater control along with AJAX and should implement masks, dropdowns, etc to make it easy for the end user. Should the user add more rows of data than one paper based form can hold, then multiple copies of the form should be created until all the data captured is printed on a digital copy of the form. 4) Many of the fields will need validation checks to be performed, such as proper data entered for phone number, date, currency, etc. These can be discussed more once the project is accepted, but you should be prepared to implement proper validations for each field on this form. When coding is completed the project is not over. We will begin user acceptance testing and the project will not be considered completed until all bugs are reported and fixed. Please realize that this may take more than one iteration based on how many things get broken in each testing/fix phase. ## Deliverables Visual Studio 2008 w/SP 1 C# or VB .Net .Net Framework 3.5 Active Reports 3.0 for .Net
