ASP.NET contact form custom server control wanted

I need a "contact form" asp.net server control written in VB.NET. I have attached a screenshot of what I would like the finished control to look like. I would like the control to raise an Event back to the hosting page and pass back the fields as well as the field values. These values should be returned via an event as a collection so we can reference the field values easily. e.g if the name input text field is called txtName and a person enters john smith as a value, then the collection returned should contain "txtName","john smith" Likewise for the other four fields. I would also like client side validation on the form (As shown in the screen shot). The name, email (email validation should look for valid email address) and message fields should all be validated before passing the information back to the server via the event. There is no need to implement styles of individual textboxes/ button/ table cells etc. Obviously, the control should be a compiled ASP.NET dll that I can add to my control bar, not a .ascx web control. Any questions, feel free to ask. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement). ## Platform ASP.NET using 1.1 framework. Visual Studio 2003 (preferred) as development environment although VS 2002 is fine. **No Visual Studio 2005 beta development please. **The finished control should be a compiled ASP.NET server control dll that I can add to my control bar and simply drag/ drop onto ASP.NET forms when I need a contact form.
