Banner Graphic / Website Template

Hi, thanks for looking at my bid request. I have a blog using blogger (html template attached in the zip file),which I would like to personalize with a stylish banner graphic. I also need a couple of links in the graphic, evenly spaced (see image attached in the zip file). And I need a signup box linking to my autoresponder in the template.
