Change Form Fields

Hello, I have an application form with around 40 fields and need to update them. I will provide you with: - the HTML for my current form. - the URL of the new form. The coder simply makes my current form the same as **all the required fields** of the new form. -------- The two forms are similar. Fields that aren't required should not be included. Required fields must be included exactly. The new form has a similar number of required fields as my current form. Form validation or scripting is not necessary, just change my form to match the new required fields. You do not have to create multiple pages, just put all the required fields on one page.
