I have a product that I sell on Clickbank.
Here is what I need to happen when a customer purchasing my product from ClickBank:
1.) The successful transaction must send the customer to an intermediate page (probably dynamically created) where they must enter their name and email address before continuing. Their email address and name will be submitted by form.
2.) After they input a successful name and email address, they will be passed to the "Thank You" page where they can download the product. This page must be protected so that people can not illictly access it directly and steal the product.
I want the form in the template of my website (I'll provide this) with three text fields. The first field is "name". The second text field is "email" and the third is "confirm email". The second and third fields must match to make sure the user has entered it correctly.
I want this done as soon as possible.
I will choose the winning coder based upon:

1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.
2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):
a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.
b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.
3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
