Directory of gas stations system.

I need someone to script a PHP/MySQL system which will allow users to go to the website, put in their zip code and prefernces (checkboxes) and search for gas station matching criteria. Any user can search based on the following: X Gas Station Name X Octane (range from X to X) Checkbox Non-Oxy (Non-Ethanol) Checkbox Diesel Checkbox Top Tier Checkbox Leaded Checkbox Unleaded Checkbox E85 X Zipcode User can create an account for free, once they have an account they will be able to post a gas station to the database. All users (including non registered users) should be able to VOTE (Like in [login to view URL] whether a post is correct or not, if the poster gets enough incorrect votes they should be not allowed to post. Basically this will be a centralized database of gas station that carry certain unique gases. A lot of car clubs and racing communities need to know where to go to get certain gases (example 100 octane unleaded or 114 octane leaded) or E85 stations... A basic user submission should include: Station Name: Address: Phone Number: Fuel Octane Rating 1: #### Checkbox Leaded/Diesal/Non-Oxi Fuel Octane Rating 2: #### Checkbox Leaded/Diesal/Non-Oxi Fuel Octane Rating 3: #### Checkbox Leaded/Diesal/Non-Oxi Fuel Octane Rating 4: #### Checkbox Leaded/Diesal/Non-Oxi (Once a user puts in a fuel type 4, there should be automatic javascript to add another fuel type, so that potentially unlimited fuel types could be added) There should be a place for Comments at the bottom of any gas station so any user can post their comment on the gas station. (For example if someone puts false info, a user should be able to post that info is false, or wrong for everyone else to see, etc...) Search results should return a list, ordered by location (Zip code locator): Name, Address, Phone, Rating (% of positive vs negative votes). Some slight changes or additions may be incurred during the creation/debugging processs. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased.
