Create a Video Lightbox for my site

I currently have a site that I am developing that has three videos posted as links. I need a programmer to modify the code on my site so that instead of those videos opening up a new page, they instead open as a light box in the middle of the existing site with the rest of the page faded in the background while the light box is open. On two pages i also need a contact form created. [login to view URL] - Add contact form at bottom of page with "refund request" as subject line. Include fields called "Order Number" , "Order Date", "First name" Last Name, Email Address, Phone Number I will send across the zipped site pages for you to update.
