Ajax/PHP/MySQL form

I'm looking for an AJAX form for entering data into a database. This will be released to a lot of people so it needs to be very robust/strong. Basically the form will sit inside Joomla. The variables that need to be sent to be stored in the database are: 1. userid of person submitting the form (people must be logged in to submit) 2. category of listing (from a predefined list) - I would like the categories to appear interactively for selection/insertion. I will provide the list of categories. 3. Title of the Project 4. Materials required to complete the project. I would like this to be linked to a standard dictionary. As people type suggestions should come up. I don't want anyone to be able to enter something that isn't in the dictionary/is misspelt etc. There is no fixed number of materials. It will also be later possible to search on materials so this will need to be structured cleverly. 5. Tools required - same as above. No fixed number of tools. Tools should be obtained from a dictionary. We may include a feature to search by tools at a later stage so bear this in mind when creating. 6. A general description of the project, no more than 500 words with 1 photo (optional). The photo must also be uploaded/stored on form submit with a unique identifier. This photo should be resized to 140x140 pixels. No more than 300kb in size (after resizing). Acceptable file types are jpg, gif, png. 7. ... I've run out of space, full requirements in attached text file. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows? (depending on the nature? of the deliverables): a)? For web sites or? other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software? installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased.
