Create aweber list, add webform, flyer and youtube video to landing page.

I have a landing page created, but need to update it with an an email capture form, graphic flyer and youtube promotional video. The form should come from aweber where you need to create a new list and copy and paste the web form code into my landing page. I have the graphic flyer ad and youtube video which should be embedded into the landing page as well. The site is custom PHP, html and javascript.
