Creating a small site to check adress against a table

I am looking for someone who can build a small site (web app) for us hopefully on a CMS system that would determine eligibility for a financial product. The user would enter a street address and the web app would turn the address into a census tract and check it against a table (We have it in a spreadsheet) if property is on list it is eligible. If it’s not it needs to check the county against an income limit that might make it qualify.
