Custom MAGENTO payment module

I need a custom MAGENTO module that works like this:
1) On a checkout process it displays custom text (configurable) and requires one text field (less than 255 chars);
2) The entered field is saved in a database and is associated with the order;
3) When the order is submitted it stays with "pending" status, and an email with invoice PDF is sent to some custom email address (configurable; This is not clients email address);
3.1) a note here is that invoice is not created, but module has to generate same looking PDF;
3.2) in the PDF there should be extra line with the text field value, which was entered during the checkout;
The module has to be not encrypted and does not connect to any remote server.
The extra table, needed for saving custom data, has to be created with install script.
If the PDF generation requires anything from the invoice (that is still not created) then this information has to be skipped.
