Code some simple PHP updates to my site (eyeintrocont)

I have one create page and one edit page that need coding. I have built the pages and even started the coding but need a professional to finish it off and make sure it all works how it should. I also have some PHP code that needs modifying slightly to go with the new settings. I have attached the details of the job along with the two files in question. Please also note that I need this job completed by the end of tomorrow. That gives you a deadline of about 1.5 days. This is important so please don't bid unless you can do this quickly.
