Amend PHP form script - machform

We have a form script called MachForm? <[login to view URL]>? which is a 'form builder' script. What this script does is allow us to create a questionnaire form which users can complete and it stores data in a mysql dbase. We are integrating this into a system we are building in PHP but one issue is that the form script doesn't allow 1 questionnaires to be split across many pages. Everything appears on one page. We need somebody to modify the code so that : 1) If there is more than 10 questions it automatically creates a pagination - so 10 questions per page 2) Then depending on the number of pages created we need a status on each page in a 'bar format to show how much percent of the questionnaire they have completed. e.g. if there were 97 questions this would be split on 4 pages so top of page 1 it will be [25%XXX ? ? ? ] page 2 [50%XXXX ? ? ] etc This is a very urgent project so please make sure before you bid you have checked the demo and are 100% confident you can complete this within 24 hours.
