Create an Online Form to Complement an Offline Form

Our Wordpress site is: [login to view URL] We have a 2 page questionnaire that we normally submit to our students at the end of the online learning with us. We would like to create an online version which would probably work as below: 1) They fill out the form 2) When they click submit its emailed to a number of us in the office ( whether as a straight email or as a pdf ( We would prefer as a pdf) This can all probably be done by a plugin.
