Add design to a fpdf document

I have the following document which is generated by results from a mysql database using FPDF. What i am looking for is for somebody to take the data and the fpdf and incorporate this design in to the fpdf document. There are some small changes for example there will be no slide bars and some of the sections will be named different, the thing for me is to have somebody build in the structure of this document to my fpdf. All information and the graphic river will be provided. All of the mysql queries are built and so all i need is literally sombody to take that data and structure the fpdf document.
