Convert classic .asp/mysql page to .php

Need this page rewritten in .php [login to view URL] The page connects with 3 mysql databases to do a zip code search and display the results. The queries, design and logic all work, just need the code converted from classic .asp and cleaned up. Current coding is older and inefficient. You will not be able to work on the live server... I'll give you the file and it will connect with 3 dummy databases to test with. So you will need your own .php environment
