Data to pdf

We have a website comparing insurance plans from several suppliers. Once they have chosen the policy they want the user can then download an application form. The application form is in the format of an editable pdf and the forms from the various suppliers are different in terms of the information requested. We want to have a standard HTML form as part of the website that the user can complete online and without leaving the site. We will then use the data from that standard form to populate the fields of the appropriate insurers pdf. We are looking for somebody to build a standard HTML form with inputs in a combination of text boxes, radio buttons, select boxes and ability to capture an electronic signature. Text needs to be large enough to be read by all users across the full range of browsers, smartphones and tablets. The form will be across several pages with data common to all forms (name, age, contact etc., on page 1). On completion of page 1, the user will select the “next” page. The data will then be saved to a mysql database and the next page presented to the user. This process will be repeated for as many times as necessary to complete all the inputs. On completion of the data input the user will need to add an electronic signature which should be captured into the database as part of an audit trail. On submission of the form, the next step will be to take the data from the mysql table and use it to populate the appropriate editable pdf. This will then be emailed to our admin centre as a completed pdf application. We will provide a sample editable pdf and would like you to 1. design a suitable HTML5 form to collect the data 2. save this to a mysql database (to be provided to us as an sql file) 3. use the data to populate the pdf fields and output a completed pdf application. We will then take the output and email it using exist email code on the site). The code needs to be sufficiently flexible and well commented to allow us to add / subtract fields to accommodate the different application forms, as well as implement future changes (suppliers tend to change their forms each year). We also need the facility to be able to translate the output from the database to other fieldnames to use with different suppliers pdf’s as there is no commonality in the naming conventions used. We would like applicants to provide details of similar projects they have undertaken previously.
