Develop a writeable PDF template from an existing document

Not sure if ihvae put this request in the correct category but lets give it a go. I currently have a word document that i wish to have developed into a writeable PDF. I think this is the correct description. The document - attached - is my current word (for mac) template i have saved. I want to make all of the fields i have highlighted fields that come up with prompts so that all my other staff can use the same template and all of our quotes go out in the same format. I have written in brackets next to the highlighted field what the prompt should be. All text that is bold in the attached doc should remain on the template. The two logos at the top and bottom of the pages are to remain. On the current document which is two pages, the address and phone number etc (Footer) only appear on the bottom of the second page. Some of our quotes are one page, some two, some more. The document should allow for this. There may be some other small changes to be made - these can be negotiated again once this first stage is completed.
