Amember-based web site design. Public area needs 3 pages: - index page - Terms of Service page - Contact Us page Index page needs the following elements: - H1, H2 both text and links - Paragraph text, links - Lists both ordered and unordered both text and links - Table, both text and link elements - Subscribe Button Contact Us page needs the form: Your Name: (required) Your Email: (required) Our Site Username: (optional) Subject: Drop-down box with the choice of subjects Message: Captcha to protect from spam-bots Submit Button On submit show the confirmation message and send email. Email addresses are different and should be configurable. Subject1 - use email1, Subject2 - use address2. Also please include user IP at the bottom of the email message. Below the form: For Mail Inquiries Use the following address: Company Name Street, # City, State, Zip Code, USA Leave Us Voice Mail: 888-888-8888 Header of all pages should feature Login/Logout link Footer should have horizontal menu with the links to Home, Terms of Service, Contact Us pages, at the very bottom are copyright notice and disclaimer text. Private area in addition should have horizontal menu below the header with the links to Download , User Manual, Page A, Page B on the left and My Account on the right Download is the default page where user gets after login in to amember account, My Account is standard amember page with user subscription/payment information. I don't have the content for the index page, Term of Service page, Page A, and Page B yet. Please put the sample with the elements requested for index page.
