add personalization optionto existing web template

I have a website template from [login to view URL] with oscommerce that I want to add a personalization option to. An example of the kind of options I want can be found here: [login to view URL] I want to be able to adjust the number of characters and number of lines depending on the product they are personalizing. I would like to be able to offer from 1 to 10 different font styles depending on the type of product they choose.
