C# SOAP generation polling application

This project consists of developing a Windows form based application in C# that will query a MSSQL database, and send resultant records via SOAP to a web service. The SOAP request consists of 4 values: username, password, base64 encoded image (PDF), and a string value. The SOAP response will be a single string value, which should be written to the same MSSQL database along with timestamp.
