cdosys form sends html email

I am putting up a website for a Church and they want a form where a user can input information that will then generate an email to be sent to a specified recipient (specified by the user) inviting them to come to church on for a certain series. And the email must contain a clickable image that can be either hosted online or sent with the email. It will look something like this below: ***FORM*** Send an invitation Your Name: (text field) Your Email: (text field) Recipients Name: (text field) Recipients Email: (text field) Note to Recipient: (text area) Send button Clear button ****END FORM*** The email sent to the recipients email address should contain all of the above information like. ***EMAIL SENT*** Hello (recipient) (Senders name) would like to invite you to come to ..... (clickable image of current series that leads to webpage) click image for more information ***END EMAIL*** And all this must work on Godaddy servers, I can provide ftp access for dev there if needed. I really dont care if it is done in something other than CDO, as long as it works and is heavily commented for possible future changes. I am currently hosting on GoDaddy Linux but can switch to Windows if needed.
