Custom Model signup form for Oxwall powered site.

I am going to need a custom model sign up form that integrates into my current oxwall signup process I need a way for models to send confidential documents (license,dob,address) and have the information forwarded to my email so i can approve them. I would also need the drop down menu from the current page removed and the sign up questions added. I can show winning bidder it a bit more detail and show some examples.
