Andriod Form App

I am looking for an experienced Eclipse Android programmer to develop an application that will take information from a form and post an XML file to a webservice with the reponses. The form will be several pages of fields; around 200 fields total. Some of the fields will need to interact with a webservice to retreieve data. Specifically, 3 fields need to lookup a person after their name is entered it will retrieve a list of possible matches (similar to blackberry name look in function). Most fields will be simple text entry, drop down or multi select radio button. If possible I would like program to include ability to attach picture file or video file from camera to report. The program would be centered around use on Motorola Xoom device with 3g access.
