Develop responsive data entry form(s) to capture the data required for sections 1, 4 and 6 of the attached Informed Consent Form, using the Porto/Porto Admin theme (theme files will be provided). Design should be adaptable to themes other than Porto. Data should be saved as JSON. Porto is at [login to view URL] The Category field (section 1) can be Employment or Volunteer - ignore the other options on the attached form. The Purpose field (section 6) must give police a clear understanding of the role - see the example sites for ways the user can be assisted to enter suitable text. It is important to establish whether the role has contact with children or vulnerable people - see the example sites for ways the user can be assisted to choose the correct value.
