Book Translation (English to Mandarin)

Am wanting an English book: http://www.amazon.com/Neal-Barnards-Program-Reversing-Diabetes/dp/1594868107/ref=pd_sim_b_3 to be translated into Mandarin (Traditional). The output needs to be in a book manualscript format as attached, in both Microsoft Word (DOC) and Adobe Acrobat (PDF) format. Translation needs to be for: a. Chapters 1-14 (179 pages) b. Appendix 2 (6 pages) I will provide a copy of the book to the winning bidder for this project, upon completion of the job, the book must be returned to me via air mail in original condition.
