My webstore needs a button and a scheme ( i have ready PSD)

My webstore is build on the shopify platform. Now i want offer my costumers to "reserve/book" a product (instead of buying it). In other words I need a button on each product page (it should automaticle appear on new products too). that links to a 3 step scheme so i get info about my costumer and what product he/she has booked. When the costumer fills out the scheme i have to get a e-mail with the info and the costumer should also get a standard e-mail telling them that we have received the info.
