Adding options for customers before adding to cart

I am having small problem, it will require very little time

Now what i want is that when a customers selects a dress, they should be given two options: Stitched and Un-Stitched
If its un-stitched, then customer can go ahead select the dress color from a drop down button
before adding it to the cart.
If the customer selects Stitched, we should give them a choice of Standard size (Small, Medium, Large) Or Custom size. If the customer selects the Standard size (Small, Medium, Large), they can go ahead select the dress color from a drop down button before adding it to the cart.
Now when the customers selects the custom size, they should be asked to fill six to seven fields of size (measurements) of that we could stitch the clothes according to their exact size.
What i want is that the customer should be able to enter their own customize size and we are prompted at the back end when we get the order. I should get all the information of what the customer purchased.
