covert PDF file into a fillable web form that info can be imported exported to and from sql

Apologize that I will not be able to describe the tech terms of the job needed. I am playing with outsourcing several things the keep my IT guy on task with internal administration programming. This is what I need... I have a PDF that I need to be able to post to my website that can be filled out and saved. This is an insurance application so there are signatures required and it ask for personal and secure information. My internal person can handle the storage of the info in a compliant manner. After saving the person that filled it out should be able to either save or print the completed document. The info that was filled in needs to be able to be extracted, saved into a my sql database. We can take it from there. Please ask any questions. I have several ( around 15) of these type docs that need to be done. I am looking for someone that can do these quickly and reasonably. I do not feel comfortable publishing the document now, but it is a final expense insurance application, They are all similar in nature.
