Create Patient Login & Medication Tracking/Order System – Wordpress Site
We have a Wordpress site and need to create a place where patients may login to access their private records and order medication.
Patients will not be able to create their own login. Patient accounts will only be created by the admin. The admin will provide the login information to the patient. Once they login, the patients will be able to modify their password.
We will provide a spreadsheet list of medications. The admin needs to be able to add, edit, and delete medication list.
Patient records should include
- First Name / Last Name
- Address / City / State / Zip / County (Patient changeable)
- Phone Number (Patient changeable)
- Date of Birth
- Drivers License Number
- Enrollment Date
- Enrollment Expiration Date
- MMMP Expiration Date
- # of Plants Designated
- Caregiver Name
- Caregiver Address / City / State / Zip / County
- Caregiver Phone Number
- Patient Medication Form – This form will be used to track the medication each patient is provided. When a patient orders medication, it should automatically be added to this form and the Admin should receive notice of the order via email. This form should include the following fields:
o Date Ordered
o Date Delivered / Picked Up (Input by Admin)
o Medication Description
o Amount
o Patient notes (Patient accessible – Patients should be able to add information to this field but may not delete past data. They can make notes on medication but will not be able to delete past notes, once they have been entered)
Patients Medication List – The admin will enter available medications for patients. The following information will need to be visible to patients. This information will be visible to patients but will only be edited by the admin.
- Product Type
- Strain Name
- Strain Type
- Strain Information (Up to 500 characters)
- THC
- CBD
- Price
- Qty (# of Ounces)
- Reviews (Written by patients, but only posted after admin approval)
Patient Order Form – Patients can order medication through their login. A patient should be able to CLICK on the product to be added to their cart. They should be able to select an AMOUNT of medicine to add to their cart. Patients will be limited to the amounts they can order.
- Patients can order up to 2.5 ounces every 15 days (1/4 Ounce at a time as the following example)
o .25 Ounce of Medicine A
o .25 Ounce of Medicine B
o .5 Ounce of Medicine C
o 1 Ounce of Medicine D
- Patients orders must total at least ONE OUNCE (Order Minimum)
The Website should TRACK PATIENT ORDERS and only allows orders to be placed as follows:
- Patients can order up to 5 Ounces per CALENDER MONTH
- Patients can only order 2.5 Ounce PER ORDER
- Patients can NOT order less than 1 OUNCE
- This will RESET 1st of every month
When the order is completed, the patient will receive a confirmation email and the admin will receive an email.
Patients will NOT pay for medicine until it is delivered or picked up. The site does NOT need any payment processing.
This site is hosted on a VPN at Hostgator and you will be given access to complete.
