I need someone to build a custom weighted quiz for my website. Each question is required and each answer is correct and has a separate weighting. Total of points at end will determine one of two results, sort of like a personality quiz but for professional purposes. Quiz consists of 5 sets of 3 to 4 questions, Radio buttons, form should also collect a minimum amount of user data, name, community, population, email and final page should display thank you and one of two results.
