Design a UI interface

For a Form generator desktop application I need a UI developer familiure with Dotnet, HTML and CSS : 1) create Only an elegant interface for a desktop application that includes 5-7 screens in C# , the developer will just create the interface matching the drawn screens but giving it an elegant view with icons and images, I should recieve the source and the exe for the application 2) I also need 5 CSS files and 1 HTML file for a slandered HR web form that should be Multi theme ((elegant, professional, smart, colorful and simple) the form should include First name, Middle Name, Last Name , email , zip code, address, CV
