create HTML form from spreadsheet Wordpress

Hi guys simple project would expect delivered today. Need a form that looks like the attached spreadsheet that will work in a Wordpress post or as a regular HTML form,need it to submit and save to mysql table that is it. Will also need script to load/edit data deliverables 1. HTML Form 2. PHP script to save data 3. PHP script that loads data from table, and allows you to edit. 3. create script for table(s)
