Develop a cloud based application

Hi, I want to collect data from multiple people at different locations. I want them to go to a webpage and fill out the data collection form everyday. The data from these forms at multiple locations will be collected and stored in a central database. The admin should be able to give people access to enter the data. The admin should also be able to see what all fields the people will enter. All this should be stored in a cloud based app which the admin and other designated users can log on to and see the data trends. I am not specific about which cloud platform you use. It should be just user friendly, clean and easy to use and edit at a later point of time.
